Back in the early 90's I was producing a NES game based on the Kevin Costner Robin Hood movie. We had to change the title screen to include the copyright symbol. What could go wrong?
Well, adding the copyright symbol made the title screen ONE BYTE larger and the data overflowed a page boundary in the NES and Boom! a few levels into the game and suddenly there was no more game.
I believe it. It doesn't happen that often. . .and even when it does, well, no one makes a change like that at the very last minute. Well, hardly ever. . .
